Welcome to Heartland

Heartland Country Village is a 50 bed not for profit skilled nursing facility licensed by the State of Wisconsin to provide skilled nursing, intermediate nursing, and rehabilitation services.  Heartland is Medicare certified and has been a part of the Black Earth community for over 60 years.

Heartland's small home-like atmosphere is conducive to providing individualized care and services.  Residents can come to Heartland for comprehensive therapies, for short term respite, or make Heartland their home for an extended stay.
