Heartland Country Village is a one story skilled care rehabilitation facility.  We are located in the quaint village of Black Earth, WI, just minutes from the larger Madison and Middleton areas.  Safety features include smoke detectors, fire rated doors, hand rails in hallways, grab bars in baths and toilets, a nursing call system, and an emergency generator.  

Heartland has 22 spacious semi-private rooms and 6 private rooms.  Home cooked meals are served in our dining room, where vaulted ceilings and a fireplace create a very home-like atmosphere.

A newly built picnic pavilion in our spacious courtyard helps our residents and their loved ones enjoy the outdoor flower and vegetable gardens.  In the winter, bird watching near one of our many feeders is a favorite past time.

There are several beautiful indoor areas as well that allow for small or large groups to gather for various activities, church services, and spending time with family and friends.
